AVMHA Registration Information
Registration Dates & Deadlines:
2012-2013 Season’s Registration starts March 15th.
Registrations taken before June 1st have the lowest cost.
Registration must be paid in full (or installment plan started) by July 15th.
“1st Time Player” would be any player that has not been registered with a Minor Hockey Association before.
How to Register:
Registration forms are available on our website www.avmha.com/wp/registration or at the AVMHA office.
Complete the form and follow the instructions on this sheet.
Take the registration form to the AVMHA office to complete (AVMHA office hours will be posted on website)
Registration nights will be advertised in newspaper and website
Registration can now be paid for online with PayPal and have the form with deposit cheques dropped off in the AVMHA office drop box as soon as possible.
Discounts: (Not eligible for Online Registration with PayPal Payment)
Goalies get 50% off the basic registration,deducted from the total registration cost. They must have their own equipment and only play the goalie position. This is only offered at the time of registration to Atom age players & older.
If after registration and the child switches to a goalie, they are welcome to use AVMHA equipment at no cost.
Family discount is for any family that has 3 or more registered in AVMHA. The youngest player will receive a 50% discount on the basic registration cost.
Fundraising Fees:
$100. fee is paid by all players at registration. Upon request, this fee may be deferred with a postdated cheque for: Jan. 31, 2013.
Female Dual Roster:
Unless otherwise indicated, female players will be registered as regular players on a co-ed team. Females can also play on a female only team (dual roster) for a cost of $100. (if there is enough female only players to form a team)
Players with 2 teams may have conflicts in scheduling from time to time and must declare in advance, which team they prefer to play with.
Rep Team Tryout:
Players must have their registration and tryout fee paid in full by July 15th. This fee is NOT refundable. Players will be guarantee 2 ice sessions.
Female Only:
Teams will be formed by division. All teams are “recreational” and no body checking allowed.
There will not be any competitive teams for Female.
PayPal
Online applications can be paid for using PayPal. You do not have to have a PayPal account if you are paying with a VISA or Mastercard. If you have a PayPal account, you can set it up to take the payment from your bank account.
Postdated Cheques for Registration:
You can have up to 6 postdated cheques to pay for Registration. (Mar. 31, Apr. 30, May 31, June 30, July 31, Aug. 15)
Other Funding:
Indicate on the Registration form if you are applying for funding to pay a part of registration. KidSport & JumpStart pays only a portion of the Registration fees. They DO NOT cover Jersey Deposits, Fundraising Fees or Tag Day Fees.
Deposits:
Jersey deposit is $150. (postdated cheque for April 1, 2013). Rep team players will be asked for another $150. to cover the 2nd jersey they are issued. If the jersey(s) is not returned or if it is returned in poor condition, the deposit(s) will be cashed.
Tag Day Deposit is $50. (postdated cheque for Nov. 30, 2012). If there is NO scheduling problem or alternate fundraising done then the cheque will be cashed.
Code of Conduct:
A copy of the player’s and parent’s code of conduct will be provided at registration. The player and the parent will be asked to sign the registration form to say they have read them.
Unpaid Fees:
Any fees outstanding from the previous year must be paid in full before we will accept registration for the new season. This includes $50. fee for a NSF cheque. If registration fees are not paid in full by the time the season starts, the player will not be allowed on the ice.
