for the 2013-2014 Season

Please open and read all documents.  To register, print the Registration Form (2 pages) and if applicable, the Female Questionaire (1 page).

Early Registration prices are no longer available.  No exceptions!  First time player discount is available at any time.

Once the forms are  completed, make out post dated cheques for deposits (jerseys & tag day).

Choices for payment:

• Take all forms to a designated Registration Night at the Multiplex (see “Events”) for payments in Cash, VISA, MasterCard, or Cheque.
• Include payment (Cash, Cheque or PayPal) and all forms in an envelope and place in Drop Box at AVMHA office.
